    Total amount of closure days may not exceed 22 days per contract year (October 1st - September 30th).

    Providers may not exceed 5 closure days per month.

    Your program's closure calendar must be submitted through this form, emailed or mailed to your Fiscal Specialist by 9/15/2017.

  • Please list any additional dates you will be closed, as well as the reason (e.g., Christmas and Spring Breaks). Please keep in mind that the total amount of closure days may not exceed 22 days per contract year, including the holidays, if any, chosen above.
